Cochlear Implant Premium Photo Medical ID Card The front includes a photo, name, date of birth, and a changeable flag. It also includes a clear message stating that the cardholder has a cochlear implant, with relevant safety details. The back includes a signature strip, a smaller photo, and another explanation of the device and safety considerations.

Cochlear Implant Photo Medical ID Card This card includes a photo, name, and a brief message stating that the cardholder has a cochlear implant which may set off alarms and must be kept away from strong magnets or scanners. The reverse includes two emergency contact names and numbers.

Cochlear Implant Medical ID Card This version does not include a photo. The front includes the cardholder’s name and a clear message about their cochlear implant and related safety information. The back provides space for two emergency contact names and phone numbers.

Cochlear Implant Mono Medical ID Card This version uses a plain black-and-white design. The front includes the cardholder’s name and a message about their cochlear implant.
